Carlson Running Club Waiver of Liability & Requirements
As a Parent/Guardian, I understand and agree to the following:
Participation in the Carlson Running Club activities can be potentially dangerous. I assume all risks associated with running while participating with Carlson Running Club, including but not limited to: falls, contact with other participants, the effects of weather (including high heat and/or humidity or extreme cold), traffic and condition of the road/sidewalk on which we will be running.
Parent/Guardian of runner is required to volunteer at Running Club at least 5 times during the school year.
Students must be picked up promptly at the Marquee in front of school no later than 3:50pm.
After two late pick-ups, students will no longer be able to participate in the Carlson Running Club.
Having read this waiver and knowing these facts in consideration of you accepting this form, I for myself and anyone entitled to act on my behalf, waive and release Carlson Running Club Coordinators, Carlson Running Club Volunteers, Alice Carlson PTA, Fort Worth Independent School District, and the City of Fort Worth, their representatives and successors from all claims or liabilities of any kind arising out of my participation with the Carlson Running Club.
